I expressly consent to the processing and transfer of my personal and health data for test kit ordering and laboratory analysis.

I expressly consent to Adon Health UG processing my personal data collected in connection with this order, including special categories of personal data as defined in Article 9 of the GDPR (in particular, health data), for the purpose of conducting the test kit, the laboratory analysis, and providing the results. This processing includes, in particular, my basic and contact information, order details, medical questionnaire data, and the results of the laboratory analysis (including testosterone, FSH, LH, SHBG, albumin, PSA, and hematocrit). I further consent to Adon Health transferring this data, to the extent and for as long as necessary for the purpose of conducting the laboratory test, to its laboratory partner Remi Health GmbH and to the laboratory actually performing the analysis, currently the MOMA Test Laboratory. I understand and agree that the aforementioned laboratories will process my personal data within the scope of their respective medical and legal responsibilities and will transmit the laboratory results back to Adon Health so that my order can be processed and the results provided to me. Without this consent, the contract cannot be fulfilled.

When using the online shop, my name, contact and address details, information about the ordered service, and technical usage data (e.g., IP address, browser information) are processed to technically enable the ordering process and fulfill my order. The online shop is operated via the Shopify platform, which processes this data on behalf of Adon Health UG.
