I consent to Adon Health processing my personal data collected in connection with this order, including health data (among other things, basic and contact information, data about the order of services from Adon Health, results of laboratory tests, e.g., but not limited to, testosterone levels, PSA, LH, etc.), for the purpose of fulfilling my contract with Adon Health and forwarding this data to our laboratory partner, Remi Health GmbH, Am Mühlenberg 11, 14476 Potsdam, in cooperation with the MOMA Test Labor, also located at Am Mühlenberg 11, 14476 Potsdam, for the purpose of conducting the laboratory tests. Furthermore, I agree that Remi Health GmbH may process my aforementioned personal data in connection with the laboratory tests and forward it to Adon Health in order to process my order with Adon Health.
